When a primary nose surgery has produced an unsatisfactory result, a revision rhinoplasty can be performed to correct the issues that exist, such as nose too large, too small, asymmetrical, collapsed nose structure or other complications. Dr. Pontell is often called upon to perform this delicate and complex surgical repair.
What is Revision Rhinoplasty?
A good candidate for revision rhinoplasty wants to correct unsatisfactory results from a previous rhinoplasty. The revision procedure may be needed for several reasons and may not necessarily be the original surgeon’s fault; rhinoplasty results partially depend on the patient’s unique anatomy and healing characteristics, and revision may also be needed to correct an injury that occurred after a primary rhinoplasty.
Rhinoplasty is a procedure performed to alter the size or shape of the nose. Due to the nose’s central location on the face, many people notice their facial harmony is affected by the appearance of their nose. Rhinoplasty is a highly requested cosmetic surgery because it can improve the balance of the facial features by creating a nose that is proportionate to the rest of the face with an attractive shape.
Rhinoplasty is one of the most complex facial surgeries, and it is one of the most requested revision procedures. Many patients unhappy with their original rhinoplasty results can take advantage of revision rhinoplasty to improve many aspects of their nose, including the shape, size, angle, and function.

The vast majority of rhinoplasty surgeries that Dr. Pontell performs are open. This means that a very small incision is made in the columella which is the bridge of skin that separates the nostrils. This small incision is connected to internal incisions which allows the skin to be separated from the nasal cartilage and bones and allows Dr. Pontell to have excellent visibility of the nasal anatomy. This allows for the nasal tissues to be rearranged and repositioned accurately into a more flattering, better-functioning position. In cases where additional cartilage or tissue is needed, Dr. Pontell will take it from a donor site such as the nasal septum or the ear.
Revision rhinoplasty recovery may take longer than the recovery from the initial nose surgery, simply due to the increased level of complexity and correction. A splint will be put over the nose for one week to help keep it straight. Patients can expect bruising to resolve over the first week and swelling to decrease gradually over the first four to six weeks. It takes greater than one year for all the swelling to resolve from a revision rhinoplasty, as opposed to about a year for an initial rhinoplasty. Most patients can go back to work in one week and return to normal activities within two weeks. Oral medication will be prescribed to help with any discomfort.
Most patients notice a significant improvement in the size and shape of their nose once the cast is removed.
Once healing is complete, patients can expect to have a nose that is more complementary to the rest of their facial profile and creates more pleasing facial harmony.
